FIBROMYALGIA SUPPORT GROUP (MEDWAY)
NEXT EVENT:OUR MAY MEETING THURSDAY
24TH MAY

Membership Renewal 2012 and our next meeting!!

Would you like to renew your membership of the Fibromyalgia Support Group (Medway), receive 6 newsletters a year, and have the opportunity of attending 6 meetings a year, as well as being able to tap into relevant information, and make contact with others with Fibromyalgia?  If so, please complete and return the form below.  If you do not wish to renew, it would be helpful if you could let me know.

Voluntary Subscription Policy

As you know, we do not, at present, have a membership subscription, but rely on voluntary donations from members, other organisations, and fundraising activities.  Voluntary donations by members can be paid at any time, but the opportunity is taken at the beginning of each year, when renewing memberships, to mention the voluntary donation policy.  If you are able to send a donation, this will be used to enable the group to continue giving the level of service it now does.  The donation can be; either a cheque (payable to Fibromyalgia Support Group (Medway)), cash (in person), or even a book of stamps.  Separate acknowledgement of all contributions is not possible unless you include an SAE, but please be assured that your contribution will be used wisely.

GIFT AID

Please note that, if you are a taxpayer, we, as a registered Charity, are able to claim back, from the Inland Revenue, 28p for every £1 you donate.  If you do pay tax, please complete the enclosed Gift Aid Form and send it back with your donation.
